Discharge Planner / Case Manager
Discharging a patient with home health services? Give us a call to fill in the gap. We are able to help with post-hospital discharge assistance and care. We provide transportation to follow-up physicians appointments and physical therapy appointments, and provide assistance with ADLs. We also help with bathing and feeding. Our caregivers are specially trained to provide support with Dementia Care.
